Start a sequence

POST 
/smartform/sequence/:sequenceId/start

Start a new sequence instance. Returns an instanceId, first question, and embed URLs for each form.

Workflow:

1. Call /start with optional email → get instanceId + first question + form URLs
2. Call /answer for each question → get next question
3. Share individual form URLs with end users (they login via OTP)
4. Workflow triggers only when sequence completes
